Study Evaluates Medtronic's Solitaire 4- X 40-mm Stent Retriever for Stroke Treatment

 

December 22, 2017—A single-center evaluation of the safety and efficacy of the long Solitaire 2 flow restoration (FR) 4- X 40-mm stent retriever (Medtronic) for large vessel occlusion (LVO) in stroke patients was published by Felix Zibold, MD, et al in CardioVascular and Interventional Radiology (2018;41:49–54).

The investigators conducted a retrospective analysis of all patients treated for acute ischemic LVO stroke with the Solitaire 2 FR device between May and October 2016 at Inselspital, University of Bern in Bern, Switzerland.

As summarized in CardioVascular and Interventional Radiology, patient-specific data were documented at baseline and discharge. Reperfusion was graded with the thrombolysis in cerebral infarction (TICI) classification. Postinterventional angiograms and follow-up cross-sectional imaging were used to evaluate complications.

The investigators reported that TICI 2b/3 recanalization was achieved in 20 of 23 (87%) patients, including 17 patients with the first retriever pass. National Institutes of Health Stroke Scale improved from a mean score at presentation of 16 (range, 4–36) to 11 (range, 0–41) at discharge. Mean modified Rankin Scale score was 3 (range, 0–6) both at discharge and 90-days posttreatment. No infarcts in other territories were observed. One patient showed a (reversible) vasospasm in the postinterventional angiogram and another patient had a small contrast extravasation in follow-up imaging.

From these findings, the investigators concluded that the Solitaire 2 FR 4- X 40-mm stent retriever is a safe and effective device for LVO acute ischemic stroke with a high recanalization rate, a low peri- and postinterventional complication rate, and a good clinical outcome. Despite potentially higher friction and shearing forces, no increased incidence of visible damage to the vessel wall was observed, noted the investigators in CardioVascular and Interventional Radiology.
